Please use this form to notify us of any planned changes in Childcare needs, including Planned absences and vacation days, as well as any permanent schedule changes. Please allow at least 2 weeks notice.

Non-Medication Consent Form

This form may be used when a parent consents to having
over-the-counter products administered to their child in a
child day care program.
An example use would be sunscreen or Diaper cream.

Individual Health Care Plan for Child with Special Health Care Needs

You may use this form or an approved equivalent to document an individual health care plan developed for a child with
special health care needs.

This form is to be completed for any child with a known allergy and must be reviewed regularly.

Little Lambs Summer Letter of Intent

Summer is on its way & our summer session begins June 30, 2025. Our hours will remain
5:30AM – 6:00PM. We are licensed to care for children ages 6 weeks through 12 years. So, if you
need childcare for your school age child, we can help. Please fill out the following information.
